29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. Production area - On or about March 13, 2019, a written energy control program (lockout/tagout) consisting of employee training, energy control procedures and periodic inspections was not available for employees exposed to the hazards of in-going nip points when performing service and/or maintenance on the furnace ovens, and other operating equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. 50 Building Production Area - On or about March 13, 2019, the wire cable and pulley that are required to open and close the doors on the furnace oven was not adequately guarded to keep employees from reaching into in-going nip points. Employees were thereby exposed to laceration and amputation hazards.
