29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Golf Course groundskeeping shop: Employees perform maintenance and groundskeeping operations that require the use of personal protective equipment. Employees used safety glasses when mixing growth regulators, which are corrosive and have a splash potential. A personal protective equipment hazard assessment has not been conducted and certified to include all tasks that would require appropriate personal protective equipment. Employees are exposed to eye damage.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Golf Course groundskeeping shop: Employees use the Dayton 8" bench grinder to sharpen tools and deburr metal edges. The left tongue guard was measured 9/16 inches from the abrasive wheel surface. Employees were exposed to struck-by hazards.
