29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   1 West Main St, South Hadley MA  On March 13, 2019 at the opening of the inspection, the employer had not implemented an energy control program to protect employees who maintained, cleaned, and set up metal working machines.  There were no written energy control procedures in use for the press brakes or other multi-energy source machines.  Employees received no training on company energy control procedures and the employer did not coordinate energy control procedures with outside repair contractors.  The employer had not performed periodic inspections of in-process energy control operations to ensure employee adherence to the written procedures and did not perform a review, between the inspector and each employee of that employee's responsibilities under each energy control procedure.
