29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On or about March 14, 2019, at the south roof edge of a residential structure under construction located at 5716 SE Pinehurst Trail in Hobe Sound, Florida, employees performing carpentry work were exposed to a fall hazard of approximately 10 feet when gaining access to and from a roof by using an extension ladder set up on an active worksite that was not secured to prevent accidental displacement.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:      On or about March 14, 2019, on the roof of a residential structure under construction located at 5716 SE Pinehurst Trail in Hobe Sound, Florida, an employee was exposed to a struck-by eye hazard when performing carpentry work using a pneumatic nail gun loaded with 2 ¼ inch steel nails while not wearing eye protection equipment.      ISCO Concrete Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1012047, citation number 1, item number 1 and was affirmed as a final order on 02/04/2015, with respect to a workplace located at 1900 NW Pine Tree Way in Stuart, Florida.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     On or about March 14, 2019, on the roof of a of a residential structure under construction located at 5716 SE Pinehurst Trail in Hobe Sound, Florida, employees performing carpentry work were exposed to a fall hazard of approximately 10 feet when installing plywood to roof trusses while not being protected from falling to the lower level.     ISCO Concrete Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1140859, citation number 1, item number 1 and was affirmed as a final order on 06/08/2016, with respect to a workplace located at 3618  SW Darwin Boulevard in Port Saint Lucie, Florida.
