29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:    a)  Co-Mail Department - On or about March 13, 2019, the employer failed to take precautions to prevent the ignition of flammable vapors from static discharge when the Bitjet printer head ground wires on print lines 2, 3, and 4 were disconnected.  The ungrounded printing equipment thereby exposed employees to injuries associated with fire.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Co-Mail Department - On or about March 25, 2019, the employer failed to ensure an energy control procedure was developed and documented for the control of hazardous energy when an employee was engaged in maintenance tasks, such as lubricating and adjusting the gear tension on the Line 2 Hytrol Conveyor.  The employee was thereby exposed to hazards associated with the unexpected energization and/or startup of the equipment.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a)  Co-Mail Department, Line 3 - On or about March 13, 2019, employees were exposed to machine hazards associated with moving parts when unjamming the Goss Loudon Stacker. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.
