29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. a. Located at 5434 Bowyer Court in Canal Winchester, Ohio: On or about March 13, 2019, the employer did not ensure that the employee performing siding work on the front of the house from a pump jack scaffold was protected from falling approximately twenty foot. The personal fall arrest system used was not properly anchored.

29 CFR 1926.451(g)(3)(i): When vertical lifelines are used, they shall be fastened to a fixed safe point of anchorage, shall be independent of the scaffold, and shall be protected from sharp edges and abrasion. Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights. a. Located at 5434 Bowyer Court in Canal Winchester, Ohio: On or about March 13, 2019, an employee was performing siding work from a pump jack scaffold approximately twenty foot from the ground. The employer did not ensure that the Super Anchor roof anchor was fastened to the roof structure in accordance to the manufactures specifications, thereby exposing the employee to a fall hazard.

29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section. a. Located at 5434 Bowyer Court in Canal Winchester, Ohio: On or about March 13, 2019, an employee was performing siding work from a pump jack scaffold approximately twenty foot from the ground. The employer did not ensure that the scaffold platform was leveled prior to performing the siding work at each elevation and pump jack poles were erected plumb to safely support the load of the scaffold platform. The condition of the scaffold exposed the employee to a fall hazard in the event of a scaffold shift or collapse.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled. a. Located at 5434 Bowyer Court in Canal Winchester, Ohio: On or about March 13, 2019, an employee was performing siding work from a pump jack scaffold approximately twenty foot from the ground. The employer did not ensure that the pump jack poles were secured at the bottom to prevent from displacement, thereby exposing the employee to a fall hazard in the event of a scaffold shift or collapse.
