29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) Mt. Calvary Baptist Church: On March 13, 2019, an employee was unloading material from a ladavator in close proximity to the unloading area of a scaffold that was approximately 19 feet high and was not protected by a fall protection system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: (a) Mt. Calvary Baptist Church: On March 13, 2019, employees were tearing off old Styrofoam from a roof while working in proximity to the rake edge that measured approximately 20 feet eave to ground without the use of a fall protection system.
