29 CFR 1910.28(b)(11)(ii): The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails that were in accordance with TABLE D-2 of this standard: a) Chip/Dust Conveyor Area: On or about March 13, 2019 and times prior, employees used the stairs equipped with 7 risers to access the conveyor areas. The stairs were not equipped with a railing on the open side facing the conveyor area.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end, and projected more than one half the diameter of the shaft: a) Dust and Chip Conveyor going to trailer: On or about March 13, 2019 and times prior, employees were exposed to a horizontal rotating shaft end that was approximately 1 inch in diameter and projected about 3 1/2 inches from the tail roller of a belt conveyor. The shaft end was approximately 1 foot above the floor level and was not protected with a shaft end guard.
