29 CFR 1910.119(f)(1)(i)(D): The employer did not develop and implement written operating procedures that provide clear instructions for emergency shutdown, including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner. At the time of the inspection, it was determined that the emergency response procedure for a chlorine release did not provide protection from exposure to unsafe levels of chlorine. The procedure for a chlorine release required employees to respond to the receiving dock where the switches for the exhaust fans in the ton container room and control room were located. The doors into the ton container room did not have appropriate moldings and gaskets to prevent leakage to the exterior and the exhaust fan in operation would direct any chlorine leak to the dock area.
