On or about March 14, 2019, and at times prior thereto, warehouse employees used unmarked, unsecured horizontal storage racks, not anchored to the floor and with a damaged vertical component. Warehouse employees were exposed to falling objects and struck-by hazards created by potential structural failure and collapse of storage rack systems.
