29 CFR 1910.30(b)(1): The employer did not train each employee in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment: a) At the rear of Building L between units 2 and 3, on or about February 11, 2019, the employer did not provide ladder safety training for an employee who was using an extension ladder to access the work while cleaning gutter guards with a long handled rake, resulting in a fall hazard.
