29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not  used:   a) Job site, 316 Ohio River Boulevard, Pittsburgh, PA: On or about March 15, 2019, during construction activities, the employer did not ensure that a safe means of scaffold access was provided on the ladder jack scaffold in that the ladder pick was located on the outside of the ladder, forcing employees to climb over it to get up/down, exposing them to a fall hazard of about 18 feet.

29 CFR 1926.451(f)(6): The clearance between scaffolds and power lines did not comply with the minimum federal safety standards:   a) Job site, 316 Ohio River Boulevard, Pittsburgh, PA: On or about March 15, 2019, during construction activities, the employer did not ensure that a ladder jack scaffold was set up at an appropriate distance from a service drop in that at least one part of the scaffold was set up about 1 foot from the scaffolding.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   a) Job site, 316 Ohio River Boulevard, Pittsburgh, PA: On or about March 15, 2019, during construction activities, the employer did not ensure that employees received appropriate safety training in scaffold safety. Deficiencies noted during the inspection included, but were not limited, lack of fall protection in use while on the scaffold, not maintaining a safe distance between the service drop and the scaffold and failing to provide a safe means of access to the scaffold.
