29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: a) 2650 Macarthur Rd. Whitehall, PA - on or about December 20, 2018, on or about December 20, 2018, an employee operating a forklift outside made a turn near the back roll-up door unaware it had been closed and as a result struck the door, but the employer had not provided refresher training after the accident had been reported. Lowes Home Center (#479) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(ii)(B), which was contained in OSHA inspection number 1215263, citation number 2, item number 3, and was affirmed as a final order on May 18, 2017, with respect to a workplace located at 774 Beal Pkwy NE, Fort Walton Beach, FL.
