29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Site- Employees operating Micro Products AD5 butt welders were not protected from an amputation hazard where the foot activated clamps were not guarded to prevent the operator from putting their hands in the point of operation. On or about March 8, 2019 an employee operating the Micro Products AD5, serial number 33787, butt welder had their hand in the point of operation for one of the two clamps when they actuated the clamp. The employee sustained two partial finger amputations.
