29 CFR 1910.22(a)(3): The employer did not ensure walking-working surfaces were maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.    The employer is failing to protect employees from a fall hazard. The steel grating on the Pugger floor was not secured when an employee fell through a floor opening in the floor to the ground below.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:    The employer is failing to protect employees from a fall hazard. The walking-working surface in the Pugger room was not inspected as necessary to prevent a floor grate from moving and creating a floor opening where an employee fell to the ground below.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  At the time of the inspection, the drive motor shaft for the rotary valve below the waste hopper in the Pugger room extended more than 1/2 the diameter of the shaft with an exposed keyway.

The guard had been removed to replace belts and had not been re-installed.
