29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a) 5009 Old Canton Rd Jackson, MS - On or about March 15, 2019 the employer exposed employees to fall hazards of approximately 14 feet to the lower level while installing plywood decking on the roof of a commercial building without adequate fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 5009 Old Canton Rd Jackson, MS - On or about March 15, 2019 the employer did not ensure that the ladders used to access a working surface approximately 14 feet above lower level was extended at least 3 feet above the access level exposing employees to a fall hazard.
