29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person: (a) Roof - On or about March 14, 2019 four employees were riding a Sky Jack SJIII 3219 scissor lift together to access the roof 15 feet 9 inches above concrete. The employees exceeded the capacity of the scissor lift, which was rated for a maximum occupancy of 2 passengers, resulting in a fall hazard.
