Detachable side protectors:    The employer is failing to ensure that all employees are protected from hazards involving injury to the eyes when operating pneumatic nail gun. This event was most recently documented at a jobsite located at or about 18517 Burdette Street, Elkhorn, Nebraska, 68022 on March 18, 2019. Three employees were working on the roof of a new residential structure, operating a pneumatic nail gun without the use of an appropriate eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system:    The employer is failing to ensure that all employees are protected from fall hazards when performing roofing operations. This event was most recently documented at the jobsite located at 18517 Burdette Street, Elkhorn, Nebraska, 68022 on March 18, 2019. Employees were working on the roof of a new residential structure without utilizing fall protection and were exposed to falls of over 22 feet to the ground below. The pitch of the roof was 6:12.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed.    The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This event was most recently documented at the jobsite located at 18517 Burdette Street, Elkhorn, NE 68022 on March 18, 2019. Employees were using a twenty-eight foot extension ladders to enter and exit the roof. This ladder did not have the side rails extended to at least three feet above the landing surface on the edge of the lower level of the roof. The ladder was extended two feet above the eve. The ground to eave height of this house measured approximately 11 feet and 7 inches.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards:    The employer failed to protect the employees from fall hazards while utilizing ladders by providing ladder training. This was most recently documented at the jobsite located at 18517 Burdette Street, Elkhorn, Nebraska, 68022 on March 18, 2019. The employer is failing to train employees to recognized fall hazards associated with the proper use and placement of ladders.
