29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): a. Hampton Inn Hotel, 116 Clairemont Ave. Decatur, GA (Jobsite) - An employee was exposed to crushing hazard. An employee was operating the Genie lull aerial forklift GTH-5519 (backing up to refuel) without being provided with the proper forklift training on this particular equipment, on or about 03/11/19.
