29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service:    Roof - When hoisting or removing material or debris to or from the roof, single use disposal bags were used on multiple occasions when they should have been removed from service.  The Employer had available unused disposal bags on the site.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    Roof - Employees were exposed on a walking/working surface were not protected from tripping in or stepping into or through depressions in the roof surface created in connection with HVAC work to be done by other trades.  Although one of the instances was marked as "hole", no protective barriers were in place.

Other anchor points were available for use.

29 CFR 1926.502(d)(16)(ii): The complete personal fall arrest system did not limit the maximum arresting force on an employee to 1,800 pounds when used with a body harness:    Roof - Of the 4 employees working on the roof at the time of inspection, one of the four was using an older fall protection lanyard that did not have a shock absorber.  The other three employees were using the proper equipment.  A proper fall protection lanyard was available on the roof to the fourth employee.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    Roof - Employees were climbing over a wall in excess of 19 inches prior to the time that 4 inches of insulation were added to the steel deck to access another level of the roof.  Equipment to provide access was available on site but not in use at the time of inspection.
