29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about at 4/11/2019, at building 6490, employees installing roof tiles on a 5:12 pitch roof without a fall protection system were exposed to an approximate 20 feet fall hazard.  b) On or about at 4/11/2019, at building 6461, employees applying mortar mix to roofing tiles on a 5:12 pitch roof without a fall protection system were exposed to an approximate 20 feet fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about at 4/11/2019, employees were using an extension ladder on the southwest side of building 6461 to access the roof that only extended about 1 1/2 feet above the upper landing surface being accessed.
