29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. a) 2nd floor: Employees used an electric powered angle grinder and an electric powered pipe threading machine for plumbing installation work. The tools were connected to an extension cord, which was routed to a nearby outlet device. The extension cord was missing a ground pin. On or about 03/19/19.
