29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about March 19, 2019, on the south side of newly built commercial building where employees were exposed to a fall hazard exceeding 10 feet when performing stucco work from fabricated frame scaffolds without each platform fully planked.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  On or about March 19, 2019, on the south side of newly built commercial building where employees were exposed to a fall hazard exceeding 10 feet while being elevated  by a JLG Skytrak without the forklift being designed by the manufacturer for such use.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  On or about March 19, 2019, on the south side of newly built commercial building where employees were exposed to a fall hazard exceeding 10 feet while being elevated  and moved in a forklift without ensuring that the platform was attached to the forks.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about March 19, 2019, on the south side of newly built commercial building where employees were exposed to a fall hazard exceeding 10 feet when performing stucco work from fabricated frame scaffolds without a portable ladder, hook-on ladder, attachable ladder, or other safe means of access.
