29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    a) 1st Floor - Dirt Ground:  Employee was exposed to electric shock while using electric tools including but not limited to an electric  Makita Circular Saw that was not protected by a GFCI or an assured equipment grounding conductor program; on or about 2/7/19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Interior of Building - Ladder Access - Ground Floor to 1st Floor: Employee engaged in inspection activities was exposed to fall hazards of approximately 13 feet while utilizing an extension ladder that did not extend at least three feet over to access the deck, on or about 2/5/19.
