29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:   On or about March 18, 2019: Employees working in the parts distribution center located at 2510 Mill Center Parkway are exposed to struck-by hazards where employees are not evaluated every three years.  The employer did not make sure that all powered industrial truck operators were evaluated every three years.
