The employees are permitted to operate forklifts with defects. On 03/19/2019 the employees operated a Toyota forklift with defects such as, but not limited to, damaged tires, missing slip resistance pad on bare metal brake pedal, and damaged seat with protruding pinch points.

The employees are permitted to operate forklifts without performing the required daily safety examination prior to operating the forklifts. On 03/19/2019 it was determined that a forklift was operated without the required safety examinations where visible defects such as, but not limited to, damaged tires, damaged seat with protruding pinch points, and bare metal brake pedal were observed.

On 03/19/2019 employees were exposed to electrical hazards where they used a quadruplet outlet with damaged two upper receptacles exposed the internal wiring of the electrical equipment.
