29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) 2056 Central Avenue, Colonie, NY - On or about 3/15/2019, an employee was installing a sign and standing on a the roof of a commercial building. The employee was not utilizing any fall protection and was exposed to a fall hazard over 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 2056 Central Avenue, Colonie, NY - On or about 3/15/2019, an employee was utilizing a ladder for access to the roof of the building. The ladder did not extend beyond the access point causing exposing the employee to a fall hazard while gaining access to the roof.
