On or about March 19, 2019, near the intersection of Katy Mills Cir and Amc Dr. in Katy, TX where employees were exposed to a struck-by hazard when working in a trench excavation without the spoil pile placed at least 2 feet from the edge of the excavation.

On or about March 19, 2019, near the intersection of Katy Mills Cir and Amc Dr. in Katy, TX where employees were exposed to struck-by hazards when installing pipes in an excavation without being protected from cave-ins by an adequate protection system.
