29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) Southwest Trench, Pioneer Crossing Project 17-530: On or about March 19, 2019 and at times prior employees were exposed to cave in hazards while installing new copper water service line at a depth of approximately 7 feet without performing a trench inspection. b) Northwest Trench, Pioneer Crossing Project 17-530: On or about March 19, 2019 and at times prior employees were exposed to cave in hazards while shooting grade at a depth of approximately 9 feet without performing a trench inspection.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Southwest Trench, Pioneer Crossing Project 17-530: On or about March 19, 2019 and at times prior, employees were exposed to cave in hazards while installing a new copper water service line in the approximately 7 foot deep trench that was improperly benched. b) Northeast Trench, Pioneer Crossing Project 17-530: On or about March 19, 2019 and at times prior, employees were exposed to cave in hazards while shooting grade in the approximately 9 foot deep trench with the North wall of the trench being nearly vertical.
