29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      (A) Guardrail systems per 29 CFR 1910.28(b)(1)(i)(A);  (B) Safety net systems per 29 CFR 1910.28(b)(1)(i)(B); or    (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems per 29 CFR 1910.28(b)(1)(i)(C).    Warehouse, Yale order/product picker: on or about March 20, 2019, employees were assigned to operate subject powered industrial vehicle with an unprotected long side of the operator's platform to pick products from inventory rack locations at heights over four feet above the adjacent ground level. Employees were exposed to fall hazards because adequate fall prevention and/or protection system was not provided and used to ensure safety.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.30(a)(3)(iv): The employer must train each employee in at least the following topics: (iv) The correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.  Warehouse, Yale order/product picker: on or about March 20, 2019, the employer failed to train operators in the correct use, adjustment, and inspection of a personal fall arrest system to prevent falls from the operator's platform. A full-body harness was provided with a lanyard that was too long to prevent falling and protecting against contact with the ground level below.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5):    Warehouse, powered industrial vehicles - on or about March 20, 2019, order picker and forklift operators were not provided with initial operator's training and/or evaluation prior to being assigned to operate the equipment. Operators using a stand-up order picker and a sit-down forklift were exposed to struck-by hazards and falls associated with unsafe operation of the vehicles likely to result in serious injury.      There is no abatement certification or documentation required for this item.

29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Northwest, warehouse, crate building area: on or about March 20, 2019, employees assigned to build shipping crates had their eyes exposed to flying debris hazard during wood and cardboard cutting operations with power cutting tools because eye protection was not provided in the workplace.  There is no abatement documentation or certification required for this item.
