29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a.) On or about March 19, 2019, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing work such as, but not limited to, preventive maintenance and repairs on automatic car washing equipment at various customer locations.

On March 14, 2019, at Meyers Auto Wash in Toledo, Ohio:   The employer did not ensure that employees were provided with adequate training on electrical safety-related work practices such as, but not limited to, the use of personal protective equipment and lock out tag out to protect against electrical hazards while working on or near energized or potentially energized electrical parts. Employees were exposed to electrical hazards while performing troubleshooting and repair work on automated car wash equipment.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    a.) On March 14, 2019, at Meyers Auto Wash in Toledo, Ohio: the employer did not ensure that employees troubleshooting car wash equipment in a motor control panel were protected from electrical shock and arc flash hazards.  Employees were not wearing personal protective equipment such as, but not limited to, hand protection while accessing the motor control panel to reset overloads.
