29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: Location: Stairs to basement On or about March 20, 2019, a set of fixed stairs that provide access to the basement area where records and supplies are kept was not provided with a midrails on both open sides of the stairwell.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition. Location: Basement walls, concrete On or about March 20, 2019, employees were required to enter a workspace to retrieve records and supplies, and the employer failed to ensure that the area was maintained in a clean and sanitary condition. Black marks were visible on areas where water leaks and where mold growth has occurred in areas such as, but not limited to, walls, floor, insulation and other surfaces that had been soaked previously due to the old foundation leaking.
