29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) On or about 03/20/2019, at the north wall of the construction site, employees used a mortar mix containing Portland cement called Rapid Set Wunderfixx. A written hazardous communications program for chemicals was not available.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition:  a) On or about March 20, 2019, at the commercial construction site, employees in an aerial lift were applying concrete mortar mix called Rapid Set Wunderfixx which contained Portland Cement without the use of personal protective equipment, such as protective gloves and safety glasses.
