29 CFR 1910.36(d)(2): Exit route doors were operated by a device or alarm that could restrict emergency use of the exit route if the device or alarm fails.    On or about March 15, 2019, and after the exit in the warehouse was restricted with a fence that required a key code to enter through the gate to the emergency exit. Employees are exposed to life safety hazards due to the restricted exit.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.134(c)(1): Where employee use of respiratory protection is required, the employer must develop and implement a written respiratory protection program that includes the provisions in 29 CFR 1910.134(c)(1)(i)-(ix).    On or about March 15, 2019, employees were exposed to respiratory stressors while being required to wear a 3M full face respirator when conducting mixing operations and handling chemicals. The employer did not develop and implement a written respiratory program that included medical evaluations, fit testing, and training.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.    On or about March 15, 2019 the employer failed to close unused openings in a control panel located in the control room exposing employees to electrical shock hazards.
