29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) Residential property at 207 Maple Ave, in Merchantville, NJ, 08109, at the front and side (South and West) of the home: Employees were exposed to falling object hazards when they performed roofing work on the lower roofs and ground level under employees working with tools overhead without protective helmets, which occurred on or about 3/20/19.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Residential property at 207 Maple Ave, in Merchantville, NJ, 08109, at the front and side (South and West) of the home: Employees were exposed to fall protection hazards when they performed roofing work on the upper and lower roofs with eve heights of approximately 25 feet and 16 feet respectively, which occurred on or about 3/20/19.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Residential property at 207 Maple Ave, in Merchantville, NJ, 08109, at the front and side (South and West) of the home: Employees were exposed to fall hazards associated with improperly setup ladders when they accessed the lower roof via extension ladder that did not extend three feet above the eve, which occurred on or about 3/20/19.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
