29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects:      The employer is failing to ensure that all employees are protected from hazards involving injury to the eyes when operating pneumatic nail gun. This event was most recently documented at a jobsite located at or about 16625 Potter Street, Bennington, Nebraska, 68007 on March 20, 2019. An employee was observed on the roof of a new residential structure, operating a pneumatic nail gun without the use of an appropriate eye protection device.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.    The employer is failing to ensure that all employees are protected from fall hazards when using fork-lifts with scaffold platforms that are not securely attached to the fork. This event most recently documented at the jobsite located at 16625 Potter Street, Bennington, NE 68007 on March 20, 2019. All employees were exposed to falls of over 14 feet and 2 inches to the ground below.

This event was most recently documented at the jobsite located at 16625 Potter Street, Bennington, Nebraska, 68007 on March 20, 2019  Employees were working on the roof of a new residential structure without utilizing fall protection and were exposed to falls of over 14 feet and 2 inches to the ground below. The pitch of the roof was 6:12.
