OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from the collapse of a work platform.    On or about March 19, 2019, and times prior thereto, employees worked from a mast-climbing work platform that did not have a means to limit upper travel of the platform.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced.    On or about March 19, 2019, and times prior thereto, employees engaged in welding work activities, while working from a mast-climbing work platform, were in use of welding cables that were damaged and had exposed bare conductors.  This exposed employees to electrical hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (General Industry reference 29 CFR 1910.27(a))    On or about March 19, 2019, and times prior thereto, the employer allowed employees to work from a mast-climbing work platform that was not fully equipped with a guardrail system and/or provided a personal fall arrest system.  This exposed the employees to fall from a height greater than 10 feet.
