On or about March 19, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about March 19, 2019, at the above addressed jobsite, employees performing carpentry operations from the second story were exposed to falls of approximately twenty eight (28) feet, created by the presence of unprotected sides.

On or about March 19, 2019, at the above addressed jobsite, employees performing carpentry operations while walking on or near the top of a mason block wall was exposed to ends of rebar that were not guarded, thereby exposing employees to the hazards associated with impalement.
