29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service. On March 10, 2019, and at times prior, for the employees exposed to struck-by hazards where adequate maintenance and inspections were not performed on a capillary sheave wheel which fell approximately 30 feet from a servicing rig at or near Well site 148-94-33D-28-4H in Mandaree, North Dakota. Abatement note: Among other feasible and useful means of abatement, the employer can follow the manufacturers recommendation to inspect the sheave before each use by removing the nut, lock pin and clevis pin; clean all the pieces and reassemble to prevent damage on the sheave wheel component or The American Petroleum Institute (API) recommended practices found in API 54, and API 8C.
