29 CFR 1910.146(c)(5)(ii)(C): Employees entered into permit spaces that met the conditions set forth in 29 CFR 1910.146(c)(5)(i) but before an employee entered the space, the internal atmosphere was not tested with a calibrated direct-reading instrument:     a) Wash Bay, entrant entered vacuum box #280764, a permit required confined space, and the attendant performed air testing with a GasAlert detector which was not calibrated in accordance with the manufacturer criteria, and was not challenged with a daily bump test per manufacturer criteria, on or about, March 20, 2019.    b) Wash Bay, entrant entered vacuum trucks including Holcombe  truck #28, a permit required space, and the attendant performed air testing with a GasAlert detector which was not calibrated in accordance with the manufacturer criteria, and was not challenged with a daily bump test per manufacturer criteria, on or about, March 20, 2019. 29 CFR 1910.146(h)(2): The employer did not ensure that all authorized entrants properly used equipment as required by 29 CFR 1910.146(d)(4)(a):  a) Wash Bay, entrant entered vacuum box #280764, a permit required confined space, and the attendant who performed the air monitoring was not aware that the monitoring instrument required either a daily full calibration or a daily functional challenge with bump gas in order to ensure that the instrument was functioning within normal parameters, on or about, March 20, 2019.  b) Wash Bay, entrant entered vacuum trucks including Holcombe truck #28, a permit required space, and the attendant who performed the air monitoring was not aware that the monitoring instrument required either a daily full calibration or a daily functional challenge with bump gas in order to ensure that the instrument was functioning within normal parameters, on or about, March 20, 2019.
