29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:  a.  On the facility property, on or about 3/21/19:  An employee must enter an in ground vault in order to provide maintenance for the water system.  The space has been designated a confined space by the employer.  The employer has not written nor developed a confined space entry program which meets the requirements of the standard.  Abatement certification must be submitted for this item.

29 CFR 1910.146(c)(5)(ii)(C): Before an employee entered a space, the internal atmosphere was not tested with a calibrated direct-reading instrument for oxygen, flammable gases and vapors and potential toxic air contaminants:  a.  On the facility property, on or about 3/21/19:  An employee is required to enter an in ground vault designated as a confined space to perform maintenance on the water system and test the backup generator.  Prior to entry, the employee did not test the internal atmosphere of the space.  Abatement certification must be submitted for this item.

On the facility property, on or about 3/21/19:  An employee was required to enter an in ground vault in order to provide maintenance for the water system and test the backup generator.  The employer had not provided training to the employee with regard to working safely within a confined space.  Abatement certification must be submitted for this item.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a.  Within the facility electrical room, on or about 3/21/19:  An electrical circuit breaker panel was missing a blank thereby exposing employees to live electrical parts.  Abatement certification must be submitted for this item.
