(b) North building of the Fairview Apartment Project: On or about March 19, 2019 and at times prior to, employees were exposed to an approximate 8-10 foot fall through a 23"x48" ladder access hole due to lack of fall protection guarding.     (c) North building of the Fairview Apartment Project: On or about March 19, 2019 and at times prior to, employees were exposed to an approximate 20 foot fall through a wall opening that had not been guarded.
