29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). The employer had not ensured that operators of dock carts, which complies with ANSI B56.8, had successfully completed training required by paragraph (l).

29 CFR 1910.178(m)(3): Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized. On March 13, 2019, in the Terminal Area near Dock Door 128, a manager operating a dock cart, had an unauthorized manager sitting on the front of the dock cart platform, an area not authorized for passengers, and the dock cart struck the rear of a stopped forklift.

29 CFR 1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. On March 13, 2019, in the Terminal near Dock Door 128, a manager was operating a dock cart with a rider sitting on the front of the cart and was traveling behind a forklift and was unable to stop in time and struck the rear of the stopped forklift.
