29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) Throughout Fairview Jobsite: On or about March 19, 2019 and at times prior to, the walking and travel paths of the jobsite were obstructed by cords, tools and building materials. Note: Abatement certification is required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Fairview Apartment Project: On or about March 19, 2019 and at times prior to, employees were receiving and installing trusses at 30 feet from ground without being secured to an anchor point for fall arrest. Note: Abatement certification IS required for this item.
