29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Woodworking Area - on or about March 22, 2019, employees performed maintenance and servicing on machinery to include blade changes to a DeltaX table saw, but the employer had not established a program consisting of energy control procedures, employee training, and periodic inspections to ensure unexpected energization did not occur.

29 CFR 1910.213(b)(1): Woodworking machine(s) were not provided with mechanical or electrical power controls so located that the operator could cut off the power from the machine without leaving his position at the point of operation:  a) Woodworking area - on or about March 22, 2019, employees operated a DeltaX table saw to perform cuts for doors manufactured, but the employees had to cut off the power by unplugging the machine due to the off switch not in operation.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed:  a) Woodworking Area - on or about March 22, 2019, employees cut wood using a Chicago Electric Power Tools 12" double-bevel sliding compound miter saw which did not have the lower guard attached, exposing the employee to the hazards of contact with the saw blade.
