Norcross GA, 30062, employees engaged in modifications to a communications tower were exposed to falling in excess of 100-feet to the ground below in that a 12-foot sling wrapped around the tower in a double choker and used as an anchor point, did not have the ends connected together to form a secure tie-off point.

Norcross GA, 30062, employees working on a communications tower were hoisted up the tower using a man basket attached to a crane, exposing employees to falling approximately 105-feet to the lower level.
