On or about March 9, 2019 and times prior thereto, employees were using an A-frame without the ladder's base being fully spread and the spreaders locked while performing repairs and torque checks to the middle of a rocket test, exposing employees to the hazard of fall from heights.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  On or about March 9, 2019 and times prior thereto, employees were working from platforms with unprotected side or edges that were 4 feet or more above a lower level to troubleshoot and perform repairs on rockets attached to test stands, exposing employees to the hazard of falling from heights.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about March 21, 2019 and times prior thereto, the following machines in the main shop area were not guarded to prevent employees' body parts from entering the point of operation during cycles, exposing employees to being struck by moving machinery hazards.  a) The Bridgeport vertical mill did not have the drill bit guarded and was used to drill holes in metal located the Building 1 weld shop. b) The HydroMech DM12 horizontal bandsaw did not have the unused portion of the blade guarded on the back side and was used to cut metal in the Building 1 weld shop.
