29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial worksite located at 10723 N Ambassador, Kansas City, Missouri. employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 18 to 20 ft fall hazard to the concrete surfaces below.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At a commercial worksite located at 10723 N Ambassador, Kansas City, Missouri, Employees were exposed to a fall hazard by using a ladder which did not extend 3 feet above the landing surface of the roof to which he gained access.
