29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 03/20/19, on the site of a 3-story house at 565 W. Delevan Ave., Buffalo, NY: Five employees engaged in roofing work in various areas on the steep roof were exposed to a fall of approx. 25 ft. No fall protection system was being used. Pitch of the roof was approx. 10 in 12. NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 03/20/19, on the site of a 3-story house at 565 W. Delevan Ave., Buffalo, NY: A Ladder Jack Scaffold was erected on the right side of the house. An extension ladder was used by employees to access and exit the scaffold. The side rails of that ladder only extended approx. 14 in. above the aluminum pik on the scaffold. NO ABATEMENT CERTIFICATION REQUIRED
