29 CFR 1926.1053(a)(24): The side rails of through or sidestep fixed ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: Staybridge Suites Site - Employees were required to access the roof using a ladder which did not extend 42 inches above the roof access level. 10 employees were on the roof working at the time and used had used this ladder to gain access.
